Before Your Visit

There are a few steps you should take prior to your visit to ensure your comfort and peace of mind and to minimize personal expense. Consider printing and completing our pre-registration form and bringing this with you on the day of your appointment.

Step 1:
It is your responsibility to know and understand the coverage provided by your insurance carrier.  Some carriers require a preauthorization before covering a test ordered by your physician at Oregon Imaging Centers.  You can find out if this is necessary by calling your insurance carrier, advising them of what has been ordered and asking if and how you can obtain a preauthorization.

Step 2:
Review the test your physician has ordered for you.  We have provided information about the experience and instructions that you must follow prior to your test. (Such as fasting, what to wear, etc.)

Step 3:
On the day of your appointment, make sure to bring a valid and current insurance card, name and address of your plan or insurer(s), name and address of your claims payer (if different from insurer), your ID and group numbers, and the name of the policyholder or subscriber. If applicable, for workers' compensation, automobile or personal injury cases, we require the name and address of your attorney, as well.

If you have questions about insurance:
Oregon Imaging Centers is an approved provider for many major insurance carriers and health plans. We will submit your claim and any secondary claims to any insurance company under the benefit coverage you specify, including workers' compensation, auto insurance and personal injury. Ultimately, you are responsible for payment of your account, including any balance not covered by insurance. We will send you a balance statement for any amount you owe after we have recieved payment by your insurance company.
