Click here to access common patient information related forms to request health records, authorization to use and disclose PHI, our Joint Privacy Practice Notice and more. We want the process for ordering studies for your patients to be easy and convenient while maintaining the highest standards of patient safety and quality. Once an order is received, our scheduling department may contact you or your patient to gather information to properly protocol the patient's imaging study.
