As of July 2019, we are implementing a Provider Portal, where registered users can login to create an order and track the appointment information.
The provider portal is intended only for Referring Provider use and does not store/give access to patient records for patient use.
Providers may opt to use the General Order Form, during the transition to the new system until is is taken offline.

A Note About Privacy:
If you prefer to print and fax the General Order Form, you can download a PDF. Please note that to ensure patient privacy, we do not accept Order Forms of any sort via email. If you want to submit a request electronically please login to the Provider Portal, or use the interactive General Order Form.
